In some remote southern regions of Malawi, it's traditional for girls to be made to have sex with a paid sex worker known as a "hyena" once they reach puberty. The act is not seen by village elders as rape, but as a form of ritual "cleansing". However, as Ed Butler reports, it has the potential to be the opposite of cleansing - a way of spreading disease. I meet Eric Aniva in the dusty yard of his three-room shack in Nsanje district in southern Malawi. Goats and chickens graze in the dirt outside. Wearing a grimy green shirt, and walking with a pronounced limp he's been lame in one leg since birth, he says , he greets me enthusiastically. He seems to like the idea of media attention. Aniva is by all accounts the pre-eminent "hyena" in this village.

The semi-structured interviews addressed sexual behavior and identity, alcohol use, and safer sex. Pain during RAI was brought up by many participants without specific prompting from the interviewer. Analysis of the interview transcripts revealed that pain was a common feature of first RAI experiences but was not limited to first-time experiences. The participants attributed pain during RAI to partner characteristics, interpersonal dynamics, lack of lubricant, and alcohol use or non-use. The main strategies participants used to address pain during RAI were setting sexual boundaries and lubricant use; a small number of participants reported purposefully consuming alcohol to prevent the pain associated with RAI. Pain can occur during anal penetration when the external anal sphincter is not relaxed; because the anus does not self-lubricate, lubricating substances are also needed to reduce pain from friction Hollows,

We have previously documented the relationships between stress and depression symptoms and adolescent women's nonuse and misuse of condoms and other contraceptive methods and on their unintended pregnancy rates. Here, we examine relationships between mental health symptoms and another understudied adolescent reproductive health behavior - frequency of sexual intercourse. We used panel data from a longitudinal, population-based cohort study of women ages 18— Weekly journals measured sociodemographic, relationship, reproductive, and mental health characteristics, sexual and contraceptive behaviors, and pregnancy. We examined 27, surveys from women during the first study year. Multi-level, mixed-effects logistic regression models estimated the relationships between stress and depression symptoms and the weekly odds of sexual intercourse while adjusting covariate fixed effects and random woman effects. Controlling for covariates, women with baseline stress symptoms had 1.
